Thank you for choosing Southeastern Cardiology. In order to decrease your wait time, please fill out the following forms and bring them with you 15 minutes before your scheduled appointment time. Please bring all medications that you currently take with you to your first appointment.

Visit our portal and create your own account where you will be able to view visit summaries, lab results, medication history and allergies. You will also be able to use your account to email your doctor directly, request refills or appointments, and more. Please note: you must complete the above paperwork if you are a new patient - the paperwork from the portal will not be valid as it does not contain pertinent information to your visit.
